Our founder, Caroline A. Weke, MSN, AGACNP-BC, is a board-certified Adult-Gerontology Acute Care Nurse Practitioner with over 10 years of hands-on experience. Her well rounded background blends clinical skill with compassionate care.
We care for adults 18 and older including seniors and those with chronic conditions through services like urgent illness care, disease management, and wellness visits. Every care plan is personalized to your life.
